Bei Morbus Crohn Patienten gibt es schon längere Zeit einen Off-Label-Use über längere Zeit mit Metro und meines Wissens hat es da bzgl. einer möglichen kanzerogenen Wirkung noch keine Probleme gegeben.

In summary, the results of this study support the absence of
an association between exposure to metronidazole for a short
time and cancer in humans and should allay concerns about
the carcinogenic potential of this widely used drug in humans.
These data should not be extrapolated to patients with pro-
longed use of metronidazole, such as some patients with
Crohn’s disease, on whom further epidemiological studies of
the carcinogenic potential of metronidazole in humans should
focus.
